Position Purpose:
This Outreach Worker position has the focus of entering data into the Homeless Management Information System (HMIS) data system as provided by the Projects for Assistance in Transition from Homelessness (PATH) team.
The Outreach Worker will be responsible for keeping up to date data in the HMIS system and will be responsible for reconciling data completeness reports.
The Outreach Worker will also participate in outreach activities in the community and assist with the coordination of care for individuals in the homeless and housing programs.
This position works directly with the data management system as well as directly with individuals in the program.
Job Requirements:
GED or High School DiplomaValid Driver's License
Qualifications:
Experience in the mental health field preferredSkilled in behavioral management techniques preferredEssential
Responsibilities:
Provide outreach and engagement services to possible eligible clientsIdentify and provide referrals to the PATH team to facilitate a person's linkage to medical and mental health servicesInteract with other providers of homeless services and community resourcesIdentify and address gaps in identified service provisionsComplete all necessary documentation within established time framesParticipate in Quality Improvement as assignedParticipate in all required workshops/seminars and attend all mandated trainingsDeliver exceptional customer service consistently to every consumerServe as a role model and demonstrate positive guest relations in representing the agencyJob Type:
